Meet your local directors
HomeCaring Vermont. Care from the heart.
I bring about 20 years as a nurse across disability, aged care and acute hospital care, and I know how complex the NDIS can be. As a registered nurse, I use my clinical skills to assess each client’s needs and provide the right support.
What matters most to me is helping people reach their goals with compassionate, respectful and person-centred support. I understand how hard it can be to find the right help, especially for people who feel overwhelmed or have limited family support. That’s why my team and I go above and beyond for every client.
I want local clients and families to feel heard, supported and confident in the care they receive. They should know they have a skilled, reliable team that understands their situation, communicates openly and works with them to achieve the outcomes that matter most to them.
Lara and the Vermont team take the time to get to know you, your health needs, cultural background and preferences, so they can match you with the right support worker and care plan.

Our approach: nurse-led and built around your goals
Our team is nurse-led, and our case managers have decades of experience between them. Several of us also understand disability and care from personal experience, not only professional experience. We plan care around each client’s goals, not around tasks. Our Case Managers stay in regular contact, follow up every concern and keep families informed. When needs change, we bring together the client, their family and the professionals around them to plan the next steps. Families tell us they notice the difference. One family described their Case Manager as “kind, caring, a great communicator and respectful”.
“As both a nurse and a parent to a child living with disability, I’ve seen firsthand how much difference the right support can make – not just for the individual, but for the whole family. At HomeCaring Vermont, we treat every person with the care, dignity and respect they deserve, and we work hard to build plans that truly reflect their lives. Supporting older Australians and people with disability isn’t just a job, it’s a privilege. I love seeing people grow in confidence, achieve goals they didn’t think possible and live life with greater independence. That’s what we aim to do every day at HomeCaring Vermont.”

Clinical oversight as part of everyday support
Our team includes a registered nurse and Endorsed Enrolled Nurses, so clients have clinical oversight of their health, medications and wound care as part of their everyday support. This sits alongside nearly 30 years of disability support coordination experience in our senior case management team. Our clients benefit from care that’s both safe and well coordinated. Every day, we support NDIS participants and Support at Home clients in their own homes across our local area in Melbourne’s eastern suburbs. We provide personal care, nursing, wound care, medication support, community access and support coordination. Our nurses and Case Managers carry out assessments in clients’ homes and build care plans around each person’s goals.
Our team is familiar with Box Hill Hospital, Maroondah Hospital, Angliss Hospital, Eastern Health Blackburn, Knox Private Hospital, Vermont Private Hospital and Eastern Health Wantirna.

Why choose HomeCaring Vermont
Care you can feel confident about.
Many families come to us after struggling to find support that works for them. These are the things they ask us about most.
-
Can I reach my Case Manager?
Every client has a named Case Manager. Our phone system connects callers directly to their own Case Manager, and an on-call team answers after hours and on weekends. One client described her Case Manager as “caring and always available”, and another family told us we were “willing to listen and respond”.
-
How quickly can supports start, and will they carry on when things change?
Our rostering team matches each client with consistent, skilled support workers and arranges cover for short-notice changes, so supports continue when needs change. We set up new services promptly. One client recently told us our team set up and delivered her supports seamlessly.
-
Will my funding stretch far enough?
We work with every client to make their funding go further. Our Case Managers review each client’s budget with them and their family and look for ways to stretch it, so clients get as much support as possible from the funding they have.
Local gaps we’re closing
- Access to support: support workers who are available when they’re needed, and new services that start without delay
